The LillyDirect 45-day refill window: how to keep the $449 Zepbound price
Short answer: on Zepbound 7.5 mg and up, LillyDirect charges $449 a month only if you complete your next refill purchase within 45 days of the day your last order was delivered. Miss it, and the same box costs $499 at 7.5 mg or $699 at 10, 12.5 or 15 mg. That is up to $250 more for one fill.

How the 45 days are counted
- The clock starts on delivery. Lilly's terms say you must complete your refill purchase within 45 days of the "delivery/received date" of your previous Zepbound vial or KwikPen order.
- What counts is when you buy, not when the new box arrives.
- Each on-time refill restarts the clock from the new delivery date.
- Lilly does not say how it counts the last day, or how a dose change is treated. Do not cut it close, and ask LillyDirect before you change dose.
Why people miss it
A Zepbound month is 28 days, so on a normal schedule you have about 17 days of buffer. People usually miss the window when something breaks the routine:
- The prescription ran out and the renewal took longer than expected.
- A pause agreed with the prescriber, for example during an illness or before a procedure.
- A trip, the holidays, or a payment that did not go through.
- Not knowing the rule existed. The TrumpRx Zepbound page shows it only after you pick a strength of 7.5 mg or more.
5 ways to stay inside the window
- Write down the delivery date the day your box arrives, or use the calculator above.
- Set a reminder for day 38. That leaves a week for problems.
- Renew your prescription early. Ask your prescriber before your last refill is used.
- Planning a pause or a trip? Talk to your prescriber first, then ask LillyDirect how pricing will apply when you restart.
- Keep your delivery emails. They are your proof of the date.
Buying the KwikPen at a retail pharmacy? Lilly's terms say the savings may not apply at the counter, and you may need to claim them through the patient portal, only for refills made within the 45 days. Claims must be submitted by March 31, 2027.
Questions people ask
Does the 45-day rule apply to Wegovy?
No. NovoCare's self-pay prices for Wegovy and the Wegovy pill do not list a refill window. See Wegovy cost.
Does it apply to the first fill?
No. The lower price applies to your first fill at a dose, and then to each refill made within 45 days.
If I missed it once, am I stuck at the higher price?
Lilly's terms do not say. Call LillyDirect before you order and ask which price applies.
Should I take my doses differently to make a box last?
No. Never change, skip or stretch your doses to manage cost without talking to your prescriber.
