Zepbound cost without insurance: price by dose

Short answer: on LillyDirect, self-pay Zepbound costs $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg to 15 mg. The $449 price only holds if you refill within 45 days of your last delivery. Miss that window and the price goes to $499 (7.5 mg) or $699 (10 to 15 mg).

What a "month" means

Lilly counts a month as 28 days: four single-dose vials or one KwikPen. Because of that, a year is about 13 fills, not 12. At $449, that is about $5,837 a year. Needles and syringes are bought separately.

Vial or KwikPen: same price

Self-pay prices are the same for the single-dose vial and the KwikPen. The single-dose pen (the autoinjector most people picture) is listed at insurance prices, not self-pay prices. Which form fits you is a choice with your prescriber.

The 45-day rule, in one line

For 7.5 mg and up, Lilly's terms say you must complete your refill purchase within 45 days of the date your previous order was delivered. If you don't, regular prices apply. That is $50 more per fill at 7.5 mg and $250 more per fill at 10 mg and above. How the window works, with a free date calculator.

Does TrumpRx have a lower price?

No. On the TrumpRx Zepbound page, you pick a strength to see the price. The prices match LillyDirect: $299 to $449 a month, with the same 45-day refill rule. TrumpRx sends you to LillyDirect to order.

If you have insurance

Check your plan's coverage first. Self-pay prices are for people paying without insurance. Lilly says savings offers have eligibility rules and monthly and yearly limits.
